1. Ensure document is complete.

   1. Name, Address, SSN, Filing Status
   2. Signature
   3. Date
   4. State: Number of withholding allowances is done.
2. If EXEMPT status is claimed: 
   1. Must be renewed by employee every February 15th, for Federal and State.
   2. If not renewed, will default to single and 0.
   3. It is the employee's responsibility to adjust this each year and route copy to FASS Payroll.
3. Save a copy in FASS employee personnel file.
4. Route completed form to Central Payroll.
